I'm also being prompted to answer the spam prevention question- even when no external link is being saved. Anyway, the screenshots below is what appears after a save (the saves have been successful even though error messages are displayed). When a file uploaded or an edited page is saved, if the page is refreshed after the error message page comes up, this prompts another file or page save identical to the one just made. That's why there are several identical "File:400steam holes deck.jpg" images and Q-jet rebuild article pages uploaded one after another.
